This is very likely because you only top up with fuel from completely empty. This additive pump system is quite dumb, because it is not important if there is too much additive in the diesel, it is important that there is not less than a certain amount - meaning, if you go and add diesel from half a tank, the pump will add additive enough for a full tank, leaving the diesel in the tank richer in additive than it is supposed to - and then the additive gets spent a lot faster. If you always go from empty to full tanks, the diesel will get the additive in the right amount, and the additive will last much longer...
