Nothing really to worry about there apart from emission equipment!

Yeah, EGR needs to look like it's not been tampered with, but I'm unclear about what's meant by the first part of the lead paragraph, maybe older diesels don't have a full cat test:

"....This inspection is only for vehicles that must have a full catalyst emissions test ... (disregarding the basic emissions test). You only need to check components that are visible and identifiable, such as catalytic converters, oxygen sensors, and exhaust gas recirculation valves."

I am not too sure how they will check that the EGR has not been tampered with. On the M59, you have to take all the pipes and fuel filter in order to see the EGR valve. Unless they are equipped with some sort of cytoscope camera.

There's the wiring & tubing back to the solenoid valve on the 2.0. But you're right there's no way to see if a blanking plate has been added.
