The heater elements in the middle part where it isn't demisting will be broken.

I've looked at the rear window but can't see any broken parts on it.

The elements can be broken but not really that visible.

If you use a multimeter on each element you can trace where the break is.

There is conductive paint available that can repair the break.

I never use my rear demister just use the air-con instead which works well.

Are the elements on the inside or outside of the rear window as I have bought a repair kit and it says about the outside of the rear window?

Heated window tracks are always on the inside. Is this kit Chinese by any chance?

Mega tip so it doesn't look really shite, is use masking tape or frog tape, after cleaning the window with vinegar or alcohol, lay two strips of tape along the top and bottom edge of one track, laid carefully straight so as to create a sharp defined gap between them, the thickness of the track. Paint the gap with the conductive paint, then while the paint is still wet, carefully remove the tape. If you wait till it's dry, it can pull the repair back off the glass.

The tracks are obviously on the inside, use the repair paint as Zion describes.
