Is this from stone cold?
Have you checked the waxstat advance unit on the rotary injection pump if it's cold starts that are an issue. They stick and fail to advance the pump / raise the idle for cold start. Makes it splutter and hunt, won't rev easily. Once warm, they're fine.
The wax stat cable (runs from the thermostat to the pump) it should be taut when cold, and slack when hot. If it's slack when cold try holding the lever it pulls on on the pump (the cold start / fast idle lever) when starting (a piece of wire will do - the engine will run fine with the lever in this position)
I jammed it at a good spot on my first 1.9D van and it worked fine for ages till I swapped it out.

No worries. It likely will be the issue then.

Yeah that looks right from memory.
It's 10 years since I had that model / engine. But it's front left as you look at the engine, near the slam panel.
See here, if this is correct the stat is item 7, follow the cable and the end of it is where the lever is:

Maybe the glow plugs are duff then, the 1.9D needs them all the time to start. No matter the temperature. Stinky exhaust will be unburned fuel. Tried a double heat before cranking?
Using a 12v lamp with wires connected, you can go from the GP rail to earth and will know when they are actually on (or supposed to be) - if it never lights up with ignition just turned on, then you might have a blown fuse or bad GP relay. If it lights up but doesn't want to fire up straight away, could be the plugs are shot. They don't last forever.
