Clutch Identification

Hello, 

I have an 08 registered Partner Mk2, the clutch is starting to slip so I am looking at replacing it myself. 

Firstly, is there a sure way to identify what clutch it might have fitted please? I note several different sizes but as the engine is a DV6ATED4 does that help? Pretty sure its not a dual mass flywheel as its always been a bit snatchy to start off in cold damp weather. 

Secondly, any tips or advice on doing the job? Will replace the final drive seals as I remember always having leaks on our old 305's after replacing drive shafts.

 Your partner would a M59 equivalent I think as the 08  year is a changeover year.
