Is This Bad workmanship or part failure (09 Clutch)

I posted regarding this issue basically I had person/mechanic install a clutch 
The install or part ! Has failed result clutch failure after 70miles 

It's been a headache from the start but I'm asking for Opinion on this bearing 

He claims bearing was separated when he removed clutch . 
Should the bearing be pressed and not separate ?  What's required to separate ? 

On the bearing are some rather flimsy looking locator tabs , the bearing can only be installed the correct way due to the rectangular locator . The bearing now has only one locator and there's some evidence of chaffing 

Would a poorly seated bearing prevent the clutch engaging ? 

What's your view on the part condition ? 

He's now asking I pay for job again
  I ordered the part and I did this to ensure it was correct , it was done by obtaining part numbers from vin @ dealer parts department . 
Your thoughts and opinions plz

If the parts have failed make a claim against the supplier or manufacturer, BUT my guess is issue is more likely to be with fitting.

are they 100% sure they fitted a new bearing with the clutch kit ?

Valeo is OE.
Looks like bearing not clipped into fork properly
